PIIINP is an abbreviation, for N propeptide of collagen type III. It is a protein that gets separated during the creation of type III collagen. Collagen plays a role in providing support to connective tissues like skin, blood vessels and organs. By measuring PIIINP levels in the bloodstream valuable information can be obtained regarding collagen synthesis (tissue remodeling).

During the synthesis of collagen type III, PIIINP is produced alongside the collagen molecule. On it undergoes cleavage and enters circulation. The levels of PIIINP reflect the process of collagen type III biosynthesis occurring in tissues at a moment. Elevated levels suggest formation of collagen while decreased levels indicate production. Measuring PIIINP proves helpful in assessing tissue damage and fibrosis related to liver, kidney, lung and heart diseases.

Since skin and blood vessels contain amounts of collagen type III PIIINP, it serves as a marker for fibrotic skin conditions like scleroderma. It also aids in evaluating remodeling associated with atherosclerosis. In cases of liver fibrosis higher PIIINP levels correspond with stages of histological fibrosis. In terms of health elevated PIIINP indicates repair processes within myocardial tissue following an injury, whereas decreasing levels over time signify recovery. Moreover, elevated PIIINP may predict outcomes for individuals with heart failure. In lung fibrosis and chronic obstructive pulmonary disease (COPD) there exists a correlation between PIIINP levels and disease severity, as progression.
