INTRODUCTION
TRACP-5b, known as tartrate-resistant acid phosphatase 5b, is an enzyme used as a biomarker to assess bone resorption activity. Secreted by activated osteoclasts during bone remodeling, measuring serum TRACP-5b levels helps quantify skeletal dynamics in normal physiology and diseases like osteoporosis.
Bone continually remodels itself through osteoclast-mediated resorption of old tissue and osteoblast formation of new bone. Osteoclast activation secretes TRACP-5b to degrade and solubilize bone mineral to initiate skeletal framework teardown. Once released into circulation, serum TRACP-5b quantifies the current rate of bone matrix catabolism by skeletal osteoclasts.
Elevated serum TRACP-5b indicates heightened osteoclastic resorption outpacing bone formation. This imbalance favoring breakdown over regeneration reflects poorly controlled skeletal remodeling that weakens bone over time. In particular, postmenopausal women exhibit increased TRACP-5b levels in line with excessive resorption from estrogen-deficient bone loss. Monitoring serum TRACP-5b allows early detection of accelerated bone loss well before fragility fractures occur. The enzyme marker gauges patient response to osteoporosis therapies as well. Anti-resorptive treatments like bisphosphonates reduce TRACP-5b levels along with slowing skeletal deterioration, affirming osteoclast inhibition.
