INTRODUCTION
Soluble α-klotho (sAKL) is a protein derived from the membrane-bound α-klotho protein. α-klotho is highly expressed in the kidney and acts as an aging suppressor. The extracellular domain of α-klotho can be enzymatically cleaved and released into circulation as soluble α-klotho. sAKL functions as a hormone that regulates activity of transporters and growth factors.
The klotho gene encodes a single-pass transmembrane protein containing a large extracellular domain. Cleavage of the extracellular portion by membrane proteases like ADAM10 and ADAM17 generates the soluble klotho fragment. sAKL can then enter the blood, urine, and cerebrospinal fluid to exert effects on distant tissues. It acts independently of membrane-bound klotho.
sAKL has been characterized as an enzyme that modifies Na+/K+-ATPase, TRPV5, and FGFR1 transporters to increase their surface expression and activity. This promotes ion homeostasis and cell sensitivity to FGF23 signaling. Through these mechanisms, circulating sAKL influences renal phosphate excretion, vitamin D synthesis, and cardiovascular function. It also provides antioxidant effects and protects against fibrosis, inflammation, and apoptosis.
