INTRODUCTION

FOLR1, known as folate receptor alpha, is a cell surface glycoprotein that internalizes dietary folic acid for critical intracellular metabolic roles. Highly expressed in epithelial cells lining folate-absorbing lumens, including prominently in certain cancer types, FOLR1 offers a portal for targeting conjugated anti-tumor pharmaceuticals.

Certain membrane-bound proteins facilitate uptake of small molecule nutrients like folic acid, which requires an extracellular-to-intracellular transport mechanism given its hydrophilic nature. By endocytosing bound vitamins, FOLR1 in epithelial membranes retrieves needed nutrients from external compartments. Once inside cells, folic acid serves as a formyl group donor to enable one-carbon transfers essential for DNA and RNA synthesis along with amino acid production, thus facilitating multiple critical metabolic pathways especially in rapidly dividing cells.

This dependency likely explains FOLR1 upregulation in over 70% of ovarian cancer and roughly 30-40% of lung and breast cancers to fuel relentless proliferation demands. By conjugating anti-tumor drugs to folic acid, medics can exploit folate receptor overexpression on cancer cells for targeted therapy delivery while minimizing off-target toxicity. Multiple cytotoxic FOLR1-directed agents now undergo clinical evaluation, with promising treatment responses yet also unique toxicity concerns.
