Peptide YY (PYY) is a 36-amino acid peptide hormone released by L cells of the distal gastrointestinal tract in response to feeding. Along with neuropeptide Y and pancreatic polypeptide, it comprises the NPY family of biologically active peptides that regulate various physiological processes. Best known as a key regulator of appetite and food intake, PYY signaling also influences gastric motility, water and electrolyte absorption, and bone metabolism.

Circulating at low fasting levels, nutrient ingestion dramatically stimulates PYY release into the bloodstream in proportion to caloric load. This activates Y2 receptors on hypothalamic neurons, suppressing the activity of orexigenic neurons coexpressing neuropeptide Y and agouti-related peptide (AgRP). Consequently, PYY elicits anorexigenic effects. Rodent studies revealed intraperitoneal PYY injection decreases food intake and body weight gain. Conversely Y2 receptor deletion causes hyperphasia and obesity, consistent with the critical role of PYY as a satiety signal. Beyond energy balance, PYY may also modulate bone remodeling based on observations of increased bone formation markers in Y2 receptor knockout mice.

However, direct translation of PYY’s metabolic benefits to humans has encountered challenges. Short-term infusion indeed decreased appetite and food intake, but chronically elevated PYY levels induced by pharmaceutical intervention failed to consistently show weight lowering effects. Nonetheless, bariatric surgery upregulating endogenous PYY production has proven effective against obesity. This highlights the importance of physiological PYY activity for energy homeostasis. Efforts continue to develop improved Y2R and PYY-based therapies that potently suppress appetite without undesirable side effects.
